Cancer/Specified-Disease Claims
To file a cancer claim or specified-disease claim, please complete the appropriate claim form and follow the guidelines below:
- Include an authorization signed and dated by the patient with every claim.
- Include the pathology report diagnosing cancer with your first claim. The hospital or doctor will furnish this report to you at your request. If the diagnosis of cancer was not made by pathological means, please be sure to submit the clinical evidence that established this diagnosis of cancer.
- Have the physician complete and sign Section B: Physician's information.
- Include a copy of your itemized hospital bill if you were hospitalized.
- Send a copy of any other bills pertaining to the claim, such as anesthesia, radiation treatments, ambulance, or nurses visits (RNs or LPNs). Please be sure that all bills are itemized and include the diagnosis, services rendered, and the actual charges.
Dread Disease Claims
To file a specified-disease claim, please complete the appropriate claim form and follow these guidelines:
- Include an authorization signed and dated by the patient with every claim.
- Send clinical documents and information used to establish the diagnosis. The hospital or doctor will furnish this information at your request.
- Include a copy of your itemized hospital bill if you were hospitalized.
- Send a completed Physician's Statement and an itemized bill showing the diagnosis, services rendered, and the actual charges for these services.
National Cancer Institute (NCI)
Refer to your policy contract to determine if you have this benefit. The details of the NCI benefit are listed below.
Aflac will pay a benefit if you need to seek evaluation or consultation at an NCI-sponsored cancer center as a result of receiving a prior diagnosis of internal cancer. The purpose of the evaluation/consultation must be to determine the appropriate course of cancer treatment.
Aflac will also pay a benefit for the transportation and lodging of the person receiving the evaluation/consultation if the cancer center is more than 100 miles from the covered person's residence.
This benefit is not payable the same day the Second Surgical Opinion Benefit is payable and is payable once per covered person.
