Dental Claims
All dental claims will need to be submitted by your dental provider on an American Dental Association (ADA) claim form.
You can assist your provider by presenting your Dental Plan ID card that was included in your policy during your next visit. If you have questions, please contact an Aflac representative 1-800-99-AFLAC (1-800-992-3522).
